Social Worker

Full Time 

Onsite - Skilled Nursing Facility 

The Social Worker provides medical social services to residents in accordance with physician orders and under the direction and
supervision of the community's Social Services Manager.  This position assesses residents' social, financial and emotional needs;
and makes approprie referrals for resolution of problems and facilites aspects of case management regarding resident transitions through the continuum of care in the community.

Qualified applicants are rqquired to possess 2 years reled experience with least 1 year of experience in a health care setting (hospital, clinic, rehabilition center, etc.).  Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution is required.  Additional requirements include excellent problem solving skills, using a customer-focused approach to problem solving and goal setting, excellent communicion and listening skills, enthusiastic, resourceful, organized and possesses strong personal awareness of nonverbal communicion. Computer skills including proficiency in MS Office.
